One may mainly struggle with voices and suspicious beliefs; another may have greater difficulty with motivation, attention or organising daily life. Symptoms also vary within the same person across time. Periods of improvement, persistent difficulties and occasional deterioration can occur in different combinations.

Schizophrenia affects several aspects of mental life, including perception, beliefs, motivation and thinking. Its presentation varies considerably. Understanding a particular feature requires considering the person’s experiences, daily functioning, physical health and wider circumstances, rather than relying on a diagnostic label alone.

Differences reflect many influences, including underlying biology, physical health, stress, substance use, treatment response and access to support. Cultural background also shapes how experiences are described and understood. A diagnostic label does not specify someone’s strengths, preferences or future. It is therefore misleading to assume that all people with schizophrenia need the same medication, living arrangements or level of supervision. Clinicians build an individual formulation: an explanation connecting symptoms, circumstances and maintaining factors. Regular review helps distinguish enduring needs from temporary problems and identifies changes that might benefit from treatment or adjustments to the person’s environment.

Individual variation also limits the usefulness of comparing people by one visible feature. Frequent voices can coexist with good functioning, while less obvious cognitive difficulties can substantially limit work. A useful description records the symptom pattern, its impact and retained strengths. This provides a more informative basis for care than assuming a fixed level of severity from the diagnosis.

Further reading: National Institute of Mental Health; World Health Organization.

Two people with schizophrenia can have very different experiences.
