Schizophrenia and schizoaffective disorder are important examples, but psychotic symptoms may also occur during mania or severe depression. Substances such as stimulants and high-THC cannabis can contribute, while prescribed medicines occasionally cause similar effects. Neurological disease, endocrine disturbances and other medical conditions also belong in the assessment.

Psychosis is a description of particular experiences and difficulties with reality testing. It can occur in several psychiatric, substance-related and medical conditions. The meaning of an experience depends on its context, associated symptoms and development over time, including the person’s cultural background.

Delirium is especially important to recognise: it involves an acute disturbance in attention and awareness, often with fluctuating confusion, and requires urgent investigation of its cause. Timing helps distinguish conditions. Clinicians ask whether symptoms occur only during mood episodes, follow substance exposure, or persist independently. Several explanations may coexist; someone with an established psychiatric diagnosis can still develop a separate medical problem. The presence of hallucinations alone cannot identify the underlying condition. Diagnosis therefore depends on the overall history, examination and appropriate investigations rather than matching one symptom to one disease.

The pattern of awareness can be particularly informative. Delirium commonly involves impaired attention and fluctuation over short periods, whereas a person with a psychotic disorder may remain alert and oriented. There are exceptions and overlap. Clinicians use this distinction to guide investigation, especially when symptoms appear alongside infection, injury or another abrupt physical change.

Further reading: National Institute of Mental Health.

Psychosis can accompany several different conditions.
