Voices are a common form in schizophrenia, although people may also hear music, footsteps or other noises. A voice may seem to come from outside the body or from inside the head. It may speak directly to the person, comment on events or resemble a conversation between several speakers.

Hallucinations are sensory experiences that occur without a corresponding external stimulus. They can affect any sense and vary in intensity, frequency and emotional meaning. They occur in several psychiatric, neurological, medical and sleep-related circumstances, so their presence alone does not establish a diagnosis of schizophrenia.

The experience varies greatly. Some voices are neutral or reassuring, while others criticise, threaten or issue instructions. Frequency and loudness do not fully determine distress; the meaning assigned to a voice and the perceived power it has can be equally important. Research examines speech processing, memory, expectations and recognition of self-generated experience. Hearing voices does not imply that a person must obey them. Assessment asks about control, coping and any pressure to act dangerously. Voices that urge harmful action, particularly when someone feels unable to resist them, require prompt professional attention and a clear safety response.

Voices can be assessed along several dimensions: location, identity, frequency, emotional tone and perceived authority. None determines the diagnosis alone. A change in distress can be meaningful even when frequency stays similar. This multidimensional approach helps identify whether treatment is reducing the experience, changing the person’s relationship with it, or improving everyday participation despite its persistence.

Further reading: National Institute of Mental Health (1); National Institute of Mental Health (2).

Auditory hallucinations involve hearing sounds without an external source.
