They can be simple, such as flashes or shapes, or complex, such as people, animals or complete scenes. They differ from illusions, in which a real object is misinterpreted, and from ordinary mental imagery, which is usually recognised as imagined rather than perceived in the surrounding world.

Hallucinations are sensory experiences that occur without a corresponding external stimulus. They can affect any sense and vary in intensity, frequency and emotional meaning. They occur in several psychiatric, neurological, medical and sleep-related circumstances, so their presence alone does not establish a diagnosis of schizophrenia.

Visual hallucinations can occur in psychosis, but clinicians also consider eye disease, neurological conditions, migraine, substances and delirium. Experiences around falling asleep or waking can occur without a psychotic disorder. Sudden visual experiences accompanied by confusion, physical illness or neurological changes deserve particular attention. Assessment explores whether the eyes are open, the lighting conditions, duration and associated symptoms. A person’s ability to recognise an image as unreal can vary and does not by itself establish the cause. Treatment follows the underlying explanation rather than assuming that every visual hallucination requires an antipsychotic medication.

Lighting, vision and level of alertness provide useful context. A shadow misidentified as a person is an illusion, while a formed image without a matching object may be a hallucination. Describing these circumstances helps guide assessment. The distinction is especially important when eyesight, sleep transitions or acute medical illness could explain an experience that initially sounds psychiatric.

Further reading: National Institute of Mental Health (1); National Institute of Mental Health (2).

Visual hallucinations involve seeing something without a matching external stimulus.
