Insight includes the ability to recognise an experience as potentially related to illness or to consider explanations other than the most immediate one. These qualities are not simply present or absent. A person may feel certain at one moment, doubt the belief later or recognise that others interpret events differently.

A delusion is a strongly held belief that is not adequately supported by evidence and is not ordinarily shared within the person’s cultural setting. Assessment considers how the belief formed, how firmly it is held and how it affects behaviour, wellbeing and relationships.

Assessment also considers preoccupation, distress and the practical influence of a belief. Reduced conviction does not necessarily mean reduced distress, and someone may function better before the belief fully changes. Insight can vary across symptoms: a person may recognise voices as a symptom while remaining convinced that surveillance is occurring. Sleep, stress and the quality of communication may affect willingness to discuss uncertainty. Psychological approaches often explore evidence and alternative explanations collaboratively. The aim is to increase flexibility and reduce harmful consequences, rather than demand an immediate admission that the person’s experience is false or meaningless.

Change can occur in stages. Someone may still endorse a belief but spend less time thinking about it, feel safer or make fewer decisions around it. These are clinically meaningful outcomes. Separating certainty, distress and behaviour allows a more accurate assessment of treatment than requiring the belief to disappear completely before any progress is recognised.

Further reading: National Institute of Mental Health.

Conviction is the strength with which a belief is held.
