It can be understandable after betrayal, discrimination or danger. In psychosis, suspicion may become unusually intense, spread to unrelated situations or persist despite evidence against it. A person may struggle to decide which people or information sources can be trusted.

Paranoia involves heightened suspicion and concern about other people’s intentions. It exists on a spectrum, from fleeting doubts to persistent beliefs of persecution. Its clinical significance depends on evidence, cultural and social context, distress, flexibility and the effect on the person’s everyday functioning.

The distinction between caution and clinically significant paranoia cannot be made from one statement alone. Clinicians ask about the events behind the concern, the degree of certainty and changes from the person’s usual thinking. They also consider whether fear is preventing shopping, appointments or contact with supportive people. Constantly scanning for danger can be exhausting and can narrow attention toward apparently threatening details. Effective support starts by understanding the feared outcome. Honest explanations, clear boundaries and predictable actions can strengthen trust. Arguments, ridicule or secretive behaviour may increase fear, even when the intention is to reassure or protect the person.

Trust is influenced by how others behave as well as by symptoms. Clear explanations of who is involved, what information is recorded and what will happen next can reduce uncertainty. Promises that cannot be kept may undermine this process. Predictability supports assessment because the person may be more willing to describe fears when the interaction itself feels understandable and respectful.

Further reading: National Institute of Mental Health.

Suspiciousness is a tendency to question whether others have hidden or harmful intentions.
