A person may move from one topic to another through links that feel meaningful internally but are not made clear. Individual sentences can be understandable while the overall conversation loses a coherent direction. The term describes a communication pattern rather than deliberate evasiveness.

Formal thought disorder concerns the organisation and expression of thinking, usually assessed through speech. It differs from the content of a belief. Clinicians interpret communication in context, considering language, culture, education, anxiety and medical conditions rather than judging an unfamiliar speaking style as inherently disordered.

Assessment considers the degree of disruption and whether it represents a change from usual speech. Wordplay, storytelling, cultural conventions or speaking in a second language can produce unexpected transitions without indicating thought disorder. Clinicians may ask how two ideas connect instead of assuming no connection exists. Loose associations can occur during psychosis and some mood states, with sleep loss or other contributors affecting severity. Supportive communication uses one topic at a time and checks understanding. Reducing pressure can help the person express an idea more clearly and prevent frustration for everyone involved.

Loose associations concern the links between ideas rather than whether the ideas are unusual. Speech may be grammatically clear while its overall direction is hard to follow. Asking the person to explain a connection can reveal meaning that was left unstated. The description should remain observational and avoid assuming that difficult communication means the person has no coherent thoughts.

Further reading: National Institute of Mental Health.

Loose associations describe speech in which connections between ideas become weak or difficult for a listener to follow.
