Severe examples are sometimes called word salad, although that expression can sound dismissive and should be used carefully. The speaker may still be trying to communicate something important and may feel frustrated when others cannot follow.

Formal thought disorder concerns the organisation and expression of thinking, usually assessed through speech. It differs from the content of a belief. Clinicians interpret communication in context, considering language, culture, education, anxiety and medical conditions rather than judging an unfamiliar speaking style as inherently disordered.

Clinicians assess the pattern while considering language proficiency, hearing, neurological illness, intoxication and altered awareness. Acute confusion or sudden language changes can indicate a medical emergency and should not automatically be attributed to schizophrenia. Assessment may involve shorter questions, non-verbal communication and information from previous functioning. The degree of speech disruption can fluctuate with illness severity, fatigue and the setting. Treatment focuses on the cause, while communication support reduces demands and allows extra time. Difficulty expressing thoughts does not prove that the person has no understanding, feelings or ability to make choices about other matters.

When communication is severely disrupted, immediate needs may have to be established through simple choices, observation and familiar supporters where appropriate. The aim is to reduce the information burden while continuing to address the person directly. Unclear speech should not be treated as permission to ignore preferences or assume that understanding is equally impaired in every area of life.

Further reading: National Institute of Mental Health.

Incoherent speech is communication whose meaning becomes very difficult to understand because words, phrases or sentences do not connect in an intelligible way.
