They may stop an activity, leave unexpectedly or react to an experience others cannot perceive. From the person’s perspective, the action may have a reason, such as avoiding a perceived threat or responding to a voice. Understanding that reason can improve assessment.

Disorganised behaviour refers to difficulty organising actions in a purposeful and context-appropriate way. It can affect ordinary routines as well as more noticeable behaviour. Interpretation requires attention to cognition, mood, physical health, culture and the person’s circumstances, rather than relying on whether an action seems unusual.

Unpredictability is a description, not proof of dangerousness. Clinicians examine triggers, sleep, mood, substances, cognition and the surrounding environment. They also consider whether the behaviour is genuinely new or simply unfamiliar to the observer. A calm setting and clear communication can make actions easier to understand and reduce escalation. When immediate safety is affected, practical boundaries and support are important. Treatment should address the underlying cause rather than rely on labels such as difficult or bizarre. Records are more useful when they describe what happened, when it occurred and what preceded it than when they offer only a judgment.

Descriptions are more reliable when they distinguish observation from interpretation. Recording that someone left a room three times is different from calling them unpredictable. The first account allows investigation of discomfort, voices, anxiety or another cause. Specific observations improve continuity because another clinician can understand what happened without inheriting an unexplained judgment about the person’s behaviour or intentions.

Further reading: National Institute of Mental Health.

Behaviour may appear unpredictable when a person’s actions change abruptly or do not seem connected to the situation.
