The mismatch may reflect disorganisation, an internally experienced event or difficulty processing social context. It does not establish what the person is feeling or whether they understand the significance of the situation.

Disorganised behaviour refers to difficulty organising actions in a purposeful and context-appropriate way. It can affect ordinary routines as well as more noticeable behaviour. Interpretation requires attention to cognition, mood, physical health, culture and the person’s circumstances, rather than relying on whether an action seems unusual.

Assessment asks about the person’s experience rather than assuming disrespect, amusement or lack of concern. Nervous laughter, cultural conventions and individual expressive styles can also create apparent mismatches. Someone responding to a distressing voice may appear distracted or emotionally inconsistent to others. Clear, non-accusatory questions can help explain the behaviour. Support focuses on understanding, reducing distress and improving communication. When responses repeatedly disrupt relationships or participation, psychological or practical assistance may help. The term should be used cautiously because social expectations vary, and behaviour that surprises an observer is not necessarily a symptom of schizophrenia or another psychiatric condition.

An emotional response may be directed toward an internal experience rather than the visible conversation. A person who smiles unexpectedly may be reacting to a remembered event or a voice. Asking gently can clarify the mismatch. This possibility should remain one explanation among several, with cultural style, anxiety and ordinary variation considered before a psychiatric interpretation is adopted.

Further reading: National Institute of Mental Health.

An inappropriate response is one that appears mismatched to the situation, such as laughing during a serious discussion or showing little visible reaction to important news.
