In schizophrenia, several factors may contribute: disorganisation, reduced motivation, depression, suspicious beliefs, cognitive impairment or adverse medication effects. Limited money, unsuitable housing and lack of access to services can create similar difficulties and must also be considered.

Disorganised behaviour refers to difficulty organising actions in a purposeful and context-appropriate way. It can affect ordinary routines as well as more noticeable behaviour. Interpretation requires attention to cognition, mood, physical health, culture and the person’s circumstances, rather than relying on whether an action seems unusual.

Assessment identifies the barrier to each activity. A person who cannot organise shopping needs different help from someone afraid that food has been poisoned. Practical support can include predictable routines, reminders, simplified choices and assistance with supplies. The person’s preferences and privacy remain important. Deterioration in eating, drinking or basic hygiene may signal worsening illness or another health problem. Severe inability to meet basic needs requires timely professional assessment. Progress should be judged against the person’s own functioning and goals rather than a rigid ideal. Effective support aims to restore agency and reduce obstacles without unnecessary criticism or control.

Self-care depends on resources as well as skills. Access to laundry, suitable food, transport and private washing facilities can determine whether a routine is achievable. Assessment should therefore examine the environment before concluding that poor self-care reflects symptoms. Practical changes may improve functioning directly, while clinical treatment addresses any additional barriers caused by beliefs, cognition, mood or medication effects.

Further reading: National Institute of Mental Health.

Disrupted self-care can involve difficulties with washing, dressing, meals, household tasks or attending to physical health.
