A person may want a different life yet struggle to begin basic tasks, attend appointments or pursue interests. Clinically this is often described as avolition. It can affect self-care, relationships, work and study without reflecting a deliberate decision to abandon these areas.

Negative symptoms describe reductions in ordinary motivation, expression and engagement. The recognised domains are avolition, anhedonia, asociality, alogia and blunted affect. They differ from positive symptoms, such as hallucinations, and can remain important even when psychosis is less prominent or has improved with treatment.

Assessment asks about both internal desire and actual behaviour. Someone may enjoy an activity once involved but find the effort of starting disproportionately difficult. Planning, expectations of reward and previous discouraging experiences can all influence engagement. Helpful support identifies achievable goals and lowers practical barriers. Progress may begin with small, repeated actions rather than a sudden return of drive. Understanding the mechanism prevents blame and helps match assistance to the difficulty. Initiation and persistence may be affected to different degrees.

Motivation can be explored through a recent goal: what the person wanted, which action was needed and where progress stopped. This separates reduced drive from difficulty organising steps or overcoming a practical barrier. The distinction supports more precise help and prevents a broad label from replacing investigation of a specific obstacle that might be changed or reduced.

Further reading: Research record 30208377; Research record 35020936.

Reduced motivation refers to difficulty generating or sustaining purposeful activity.
