It is often called blunted affect. The visible expression of emotion and the internal experience of emotion are separate processes. Someone may feel affection, sadness or excitement strongly while showing little of it outwardly.

Negative symptoms describe reductions in ordinary motivation, expression and engagement. The recognised domains are avolition, anhedonia, asociality, alogia and blunted affect. They differ from positive symptoms, such as hallucinations, and can remain important even when psychosis is less prominent or has improved with treatment.

Assessment examines several forms of expression and compares them with the person’s usual style. Medication-induced movement changes, depression and cultural conventions can influence presentation. Supporters can avoid misunderstandings by asking about feelings rather than assuming that a neutral face signals indifference. Pressure to smile or appear enthusiastic may increase discomfort without changing the underlying difficulty. Treatment and rehabilitation should consider communication and relationships, while respecting the person’s expressive style and avoiding judgments about their character or capacity for attachment.

Emotional expression is assessed across face, voice and body movement. These channels may be affected differently, so a neutral face can coexist with warmth in words or gestures. Recording the separate features gives a more accurate description than calling someone emotionless. The person’s report remains necessary when understanding what they actually feel during the interaction or event.

Further reading: Research record 30208377; Research record 35020936.

Reduced emotional expression includes less facial movement, fewer expressive gestures and a voice with limited variation.
