A person with avolition may know that washing, shopping or contacting a friend matters but remain unable to get started. The obstacle can feel difficult to explain because the necessary physical skills are still present and the intention may be genuine.

Avolition is a reduction in the initiation and persistence of goal-directed activity. It is one recognised negative symptom of schizophrenia, although similar difficulties have other causes. It affects the translation of intentions into actions and cannot be understood simply by observing whether a person appears busy.

Assessment asks what happens immediately before an activity is abandoned. Anticipated effort, uncertainty about the first step and weak expectations of reward may contribute. Support can make the first action concrete, reduce decisions and provide a predictable cue. Examples include placing supplies together or agreeing on a brief starting time. Success should be evaluated by whether the strategy helps the person begin, rather than by how simple the task seems to someone else. An achievable first step can be more useful than a distant target.

Starting can be made easier by reducing the number of decisions required. Preparing materials or agreeing on the first small action makes the task more concrete. This is an environmental adaptation rather than a judgment about character. Its usefulness is demonstrated when the person can begin more reliably and retain ownership of the activity and its purpose.

Further reading: Research record 33637748; Research record 30208377.

Initiation is the step between intending to do something and beginning it.
