Depression commonly includes sustained low mood, hopelessness, guilt or negative self-evaluation. Avolition can occur without these experiences. Someone may feel emotionally neutral or still want enjoyable experiences while struggling to initiate the actions needed to reach them.

Avolition is a reduction in the initiation and persistence of goal-directed activity. It is one recognised negative symptom of schizophrenia, although similar difficulties have other causes. It affects the translation of intentions into actions and cannot be understood simply by observing whether a person appears busy.

The two can coexist, making careful assessment essential. Sleep, appetite, mood, pleasure, energy and thoughts about the future help build the picture. Medication effects and physical illness must also be considered. Treating reduced activity as automatically depressive may miss a negative symptom, while assuming it is avolition may overlook treatable depression. The distinction relies on the person’s experience and course over time, not a single outward sign. Support should address all contributing factors rather than force the difficulty into one category.

The emotional tone accompanying inactivity is informative. Persistent sadness, guilt or hopelessness may suggest depression, while reduced initiation without those feelings may fit another explanation. Neither pattern is conclusive alone. Asking about recent examples and comparing changes over time gives a more reliable picture than deciding from visible activity, facial expression or the person’s diagnosis alone.

Further reading: Research record 33637748; Research record 30208377.

Depression and avolition can both involve reduced activity, but they are not identical.
