Limited awareness of illness can make engagement difficult when the person does not see a reason for treatment. Side effects, previous coercion, practical barriers and disagreement about goals can create similar difficulties and should be explored separately.

Anosognosia describes impaired awareness of an illness or its effects. In schizophrenia, awareness can vary across symptoms and over time. It is not equivalent to a deliberate refusal to accept obvious facts, and disagreement about treatment does not by itself establish anosognosia or an inability to make decisions.

Clinicians can begin with concerns the person recognises, such as poor sleep, frightening experiences or difficulty returning to work. Explaining options honestly and following through on agreements supports trust. Engagement should not be measured only by medication acceptance or use of diagnostic language. A person may participate meaningfully while holding a different explanation of their experiences. Clear crisis plans and agreed contact arrangements can help when symptoms worsen. The relationship itself is an important part of care, especially when understanding changes over time.

Engagement has several dimensions, including attending, communicating, weighing options and seeking help when needed. Measuring it only through medication use can miss these contributions. A shared goal may create a useful starting point even when diagnosis is disputed. Practical barriers and adverse effects should be addressed openly so that disagreement is not automatically interpreted as impaired awareness.

Further reading: National Institute of Mental Health.

Treatment engagement means maintaining a workable relationship with services and participating in decisions about care.
