These changes are non-specific: they are also common in many other conditions and during stressful periods. Their presence cannot identify schizophrenia or establish that psychosis will develop. The person’s broader circumstances are essential to interpretation.

A prodrome is a period of changes that precedes a more clearly defined illness episode. In psychosis, early changes may involve functioning, mood, sleep or unusual experiences. The term is often applied retrospectively because many similar changes occur in people who never develop a psychotic disorder.

Assessment considers sleep, energy, self-worth, relationships and functioning alongside any unusual experiences. Treating anxiety or depression may reduce distress regardless of future diagnosis. Supporters can notice change without interpreting every emotion as an early warning sign. Questions about how the person is coping are often more useful than close surveillance. If changes persist or interfere with ordinary life, professional assessment can help clarify needs. A flexible plan allows care to evolve as the pattern becomes clearer rather than making one emotional change carry an excessive diagnostic meaning.

Emotional change is informative when considered with context and duration. Anxiety after a major event may be understandable, while persistent deterioration across sleep, concentration and functioning may call for broader assessment. No single feeling distinguishes a prodrome. The term becomes more meaningful retrospectively, after the later course shows whether the early changes were related to a psychotic episode.

Further reading: National Institute of Mental Health; NICE clinical guideline.

Anxiety, low mood, irritability or emotional withdrawal can appear before a first episode of psychosis.
