Schizophrenia is one possibility, but mania, severe depression and schizoaffective disorder may also involve psychosis. Intoxication, withdrawal and some prescribed medicines can contribute. Neurological and other medical illnesses require consideration when the presentation suggests them.

First-episode psychosis describes the first recognised period of clinically significant psychotic symptoms. It is a starting point for assessment rather than a final diagnosis. Different causes can initially look similar, and the person’s needs may include emotional reassurance, medical evaluation and support with disrupted education, work or relationships.

Timing and associated features help guide assessment. Fluctuating attention, fever, seizures or sudden neurological changes can point toward urgent medical investigation. The relationship between psychosis and mood symptoms may become clearer only with follow-up. Several contributors can coexist, so substance use does not automatically explain everything. An initial broad description can be more accurate than an immediate specific diagnosis. Treatment should not be delayed unnecessarily while uncertainty remains, but the rationale for care and the questions still being investigated should be explained clearly.

The relationship between symptoms and exposures is often reconstructed over time. Improvement after a substance clears or a mood episode settles can be informative, but does not answer every diagnostic question. Follow-up allows clinicians to examine recurrence and functioning between episodes. This longitudinal information complements the initial investigation and can refine the explanation without invalidating the care already provided.

Further reading: National Institute of Mental Health (1); National Institute of Mental Health (2).

The causes of a first psychotic presentation include psychiatric, substance-related and medical conditions.
