It may include medication review, psychological therapy, sleep support, substance-use care and practical assistance. The balance depends on previous episodes, remaining symptoms, treatment effects and personal preferences. No single strategy guarantees that relapse will not occur.

Relapse means a significant return or worsening of illness symptoms after a period of improvement. Definitions vary across clinical services and research studies. A brief fluctuation is not automatically a relapse, and changes should be interpreted against the person’s usual pattern, functioning and circumstances rather than one isolated sign.

An individual plan identifies helpful routines, early changes and agreed responses. It should also address barriers such as transport, cost, side effects or difficulty contacting services. Reviewing the plan after an episode can reveal what worked and what needs adjustment. Prevention should support a satisfying life rather than restrict every activity out of fear. Shared decisions make plans more realistic and easier to use. Continuity of care and clear communication matter because early concerns are only useful if they lead to accessible and appropriate help.

Prevention includes making treatment workable. Side-effect management, prescription access and a plan for changing circumstances can matter as much as information about relapse. A person is more able to use a plan that fits their life and addresses difficulties honestly. Review should therefore ask about feasibility and preferences, not simply whether instructions were understood or followed exactly.

Further reading: National Institute of Mental Health; NICE clinical guideline.

Relapse prevention combines effective ongoing treatment with attention to the person’s wider circumstances.
