Clinical recovery may include sustained symptom improvement and functioning. Personal recovery concerns building a meaningful life, identity and sense of agency, sometimes while symptoms remain. A person can meet remission criteria without feeling recovered or experience substantial personal recovery without complete symptom absence.

Remission describes a period in which symptoms are absent or reduced below a defined level. It is a clinical description rather than a guarantee about the future. Different studies and services use different thresholds, so the meaning should be explained whenever remission is discussed as a treatment outcome.

The terms should therefore be defined rather than used interchangeably. Their distinction helps prevent narrow judgments about success. Someone may value returning to a relationship more than reaching a particular symptom score. Another may prioritise relief from an intrusive experience before considering work or study. Care can support both clinical improvement and personal goals. The person’s perspective is essential when evaluating outcomes, and a remaining symptom should not automatically negate progress in confidence, participation or quality of life.

Personal recovery can include identity and agency after illness. These are not fully represented by a symptom threshold or employment measure. Asking what feels meaningful gives the person a role in defining success. Clinical remission can support that process, but it should remain one part of a broader account of wellbeing, participation and the life the person wants to build.

Further reading: National Institute of Mental Health; Research record 15741458.

Remission usually refers to symptoms, while recovery has broader meanings.
