The hypothalamic–pituitary–adrenal axis helps regulate cortisol, while sympathetic activation contributes to changes in heart rate and alertness. These systems adapt behaviour and physiology to perceived demands.

Stress is the response to demands or threats that challenge a person’s resources. It involves psychological experience, bodily changes and social circumstances. In schizophrenia, stress may influence symptoms and functioning, but it is not a complete explanation of the illness. The same situation can affect different people in different ways.

Researchers investigate whether stress-response patterns differ in schizophrenia and how they relate to dopamine, inflammation and symptoms. Findings vary, and a cortisol measurement is not a diagnostic test for schizophrenia or a complete measure of someone’s stress. Sleep, medication, physical illness and the timing of sampling affect results. Biological research complements the person’s account rather than replacing it. Understanding stress systems can explain why psychological pressures have bodily effects, but it should not encourage the assumption that one hormone level reveals the cause of a person’s experiences or identifies a universal treatment.

Stress systems follow rhythms and respond to context. A single hormone sample can be affected by time of day, recent activity and the sampling procedure itself. Research therefore uses carefully designed measurements and comparisons. This explains why a laboratory result cannot simply confirm or dismiss someone’s report of chronic strain or identify the exact source of their distress.

Further reading: National Institute of Mental Health; Research record 25688430.

The stress response involves coordinated activity across the brain, autonomic nervous system and hormonal systems.
