People with schizophrenia can experience these events just as others do and may face additional vulnerability through social exclusion or unsafe circumstances. Trauma can affect sleep, trust, arousal and the interpretation of threat.

Trauma refers to experiences involving serious threat, harm or overwhelming distress, and the effects those experiences can have. Trauma exposure is associated with psychosis in research, but it is neither necessary nor sufficient for schizophrenia. People differ in their responses, and experiences should be discussed sensitively without assumptions or pressure.

Assessment examines new changes rather than automatically attributing every difficulty to the existing diagnosis. Post-traumatic symptoms may coexist with psychosis and require specific care. The person’s account should be taken seriously, including reports of real victimisation. Support begins with safety and access to appropriate medical, psychological and practical assistance. Disclosure should not be forced. Understanding adult trauma also means recognising that services themselves can feel threatening, so explanations, choice and respectful communication can reduce additional distress while necessary assessment or treatment takes place.

A previous diagnosis should not make reports of new harm less credible. People experiencing psychosis can also be victims of assault or exploitation. Assessment should examine the reported event appropriately and separate established facts, uncertain details and symptom-related interpretations. This approach protects access to care and avoids allowing a psychiatric label to obscure a genuine safety or medical concern.

Further reading: Research record 22461484; National Institute of Mental Health; Research record 23599019.

Traumatic experiences in adulthood may include assault, accidents, violence, disasters or frightening healthcare experiences.
