Studies link trauma exposure with psychotic experiences, but association alone does not reveal the complete causal pathway. Shared risks, measurement differences and the effects of emerging illness can complicate interpretation.

Trauma refers to experiences involving serious threat, harm or overwhelming distress, and the effects those experiences can have. Trauma exposure is associated with psychosis in research, but it is neither necessary nor sufficient for schizophrenia. People differ in their responses, and experiences should be discussed sensitively without assumptions or pressure.

Proposed mechanisms include altered threat processing, dissociation, stress sensitivity and beliefs about self and others. Different pathways may apply to different people. Research across multiple designs strengthens understanding, but it cannot reconstruct the cause of one person’s illness with certainty. Clinical care can still address trauma-related distress without settling every causal question. Explanations should avoid both extremes: dismissing trauma because biology matters, or treating trauma as the sole explanation for all psychosis. A multifactorial account leaves room for the person’s experiences and for other relevant health needs.

Causal pathways may differ between people exposed to similar events. Threat expectations, disrupted sleep, dissociation and social consequences are among the processes investigated. Identifying a relevant process can help treatment without requiring proof that trauma explains the entire illness. This keeps clinical work focused on current needs while recognising that research models describe possibilities rather than complete personal histories.

Further reading: Research record 22461484; National Institute of Mental Health; Research record 23599019.

An association means that two factors occur together more often than expected under a particular comparison.
