Many also affect serotonin, histamine, adrenergic or muscarinic receptors. These additional actions help explain differences in sedation, appetite, blood pressure and other effects, although receptor binding cannot predict every clinical outcome.

Antipsychotics are medicines used to treat psychotic symptoms and, for some drugs, other conditions. Most established agents act through dopamine D2 receptors, with substantial differences in additional targets and pharmacology. Benefits and adverse effects vary between medicines and individuals, so treatment is assessed through observed outcomes rather than the drug’s label alone.

A newer muscarinic approach illustrates that direct D2 blockade is not the only possible route to antipsychotic efficacy. Mechanisms remain incompletely understood even for familiar medicines. Affinity in a laboratory assay differs from receptor occupancy and from functional effects after binding. Active metabolites can add further actions. Comparing medicines therefore requires both pharmacology and clinical evidence. A broader receptor profile is not automatically more effective, and a selective profile is not automatically free from adverse effects. The relevant balance must be evaluated for the person taking treatment.

Xanomeline–trospium illustrates an alternative approach involving central muscarinic receptor stimulation with a peripheral blocker intended to reduce some unwanted effects. Its US approval in 2024 showed that direct D2 blockade is not required for every effective antipsychotic strategy. The approach has its own risks and evidence base, so mechanistic novelty should be distinguished from proven superiority over existing options.

Further reading: National Institute of Mental Health; Research record 10739409; DailyMed prescribing information.

Most established antipsychotics reduce D2-mediated dopamine signalling through antagonism or partial agonism.
