Depending on the medicine and circumstances, this can include weight, blood pressure, glucose, lipids, movement symptoms and hormonal concerns. An electrocardiogram or additional tests may be appropriate when particular risks are present.

Antipsychotics are medicines used to treat psychotic symptoms and, for some drugs, other conditions. Most established agents act through dopamine D2 receptors, with substantial differences in additional targets and pharmacology. Benefits and adverse effects vary between medicines and individuals, so treatment is assessed through observed outcomes rather than the drug’s label alone.

Monitoring should also ask about subjective effects such as restlessness, sleepiness, sexual difficulties, constipation and emotional changes. A normal blood test does not rule out a distressing adverse effect. Some drugs require specific monitoring, especially clozapine. New severe symptoms, including marked physical deterioration, require timely assessment rather than waiting for a routine appointment. The purpose is to identify problems early and guide shared decisions. Results should be explained clearly, with an agreed plan for follow-up, so monitoring becomes useful care rather than a collection of unexplained measurements.

Monitoring is useful when results lead to action. A change in weight, glucose, movement or subjective wellbeing should prompt an explanation and a plan proportionate to the finding. Different medicines require different checks, and a routine panel is not an exhaustive safety assessment. Direct questions about daily functioning and uncomfortable effects remain necessary alongside measurements and laboratory results.

Further reading: National Institute of Mental Health; Research record 10739409; DailyMed prescribing information.

Safety monitoring checks for adverse effects and changes in physical health during antipsychotic treatment.
