They belong to different chemical families and are available in different oral or injectable forms depending on the medicine and country. Their shared classification does not mean that one can be substituted for another without clinical planning.

First-generation antipsychotics are an established group of medicines whose antipsychotic action predominantly involves dopamine D2 blockade. They differ substantially in additional receptor effects, dose requirements and formulations. The terms typical and first generation describe a broad family, not a uniform experience of treatment or a ranking of clinical usefulness.

Haloperidol has prominent D2-mediated effects, while chlorpromazine has more substantial antiadrenergic and anticholinergic actions. These differences influence adverse effects and the doses used. Some people respond well to a first-generation medicine and prefer its overall balance to alternatives. Others find particular effects difficult. Comparisons should specify the individual drug rather than assume that all older medicines are ineffective, heavily sedating or identical. Product information and local prescribing guidance remain important because indications and formulations differ across jurisdictions.

Formulation changes the practical experience of a medicine. An oral preparation and a long-acting injection may contain related active drug but differ in release, persistence and follow-up requirements. These differences should be considered alongside receptor effects. Choosing an established medicine can be reasonable when its individual benefits and burdens fit the person’s needs and preferences better than available alternatives.

Further reading: DailyMed prescribing information (1); DailyMed prescribing information (2).

Examples of first-generation antipsychotics include haloperidol, chlorpromazine, fluphenazine and several related agents.
