Their effectiveness helped establish that psychosis could be treated pharmacologically. They remain useful options for some people, including those with a good previous response or a preference for an available formulation.

First-generation antipsychotics are an established group of medicines whose antipsychotic action predominantly involves dopamine D2 blockade. They differ substantially in additional receptor effects, dose requirements and formulations. The terms typical and first generation describe a broad family, not a uniform experience of treatment or a ranking of clinical usefulness.

Benefit varies and should be assessed across specific symptoms and daily functioning. Sedation or reduced visible agitation is not sufficient evidence that the underlying psychosis has improved. Negative and cognitive symptoms may remain, and adverse effects can limit practical benefit. A medicine that works well for one individual may be unsuitable for another. Comparisons with newer agents should use clinical evidence and the person’s priorities rather than age of introduction alone. Ongoing review considers whether the treatment continues to provide meaningful relief at an acceptable physical and subjective cost.

A previous good response is useful evidence in treatment planning, though circumstances and physical health can change. Review should establish which symptoms improved, how long the benefit lasted and which unwanted effects occurred. This history provides a more personalised basis for comparison than assuming that an older medicine is suitable or unsuitable solely because of its generation.

Further reading: DailyMed prescribing information (1); DailyMed prescribing information (2).

First-generation antipsychotics can reduce hallucinations, delusions and disorganised thinking.
