Some classifications also include D2 partial agonists within the second generation, while others describe them separately. The grouping is therefore useful historically but less precise than naming the drug and its mechanism.

Second-generation antipsychotics are a diverse group often called atypical antipsychotics. Most combine dopamine-related effects with substantial serotonin receptor actions, but there is no single receptor profile shared by every member. Their introduction expanded treatment options without eliminating the need to assess each medicine’s benefits, physical effects and subjective tolerability.

Individual agents differ in approved indications, formulations, metabolism and adverse effects. Clozapine has a distinctive role in treatment-resistant schizophrenia, while other agents may have evidence for particular mood conditions. These indications are not interchangeable. Availability also differs between countries. An encyclopedia entry should therefore identify general pharmacology without assuming that every reader can obtain every medicine or use it for the same purpose. Clinical comparison needs the exact drug, formulation and treatment goal, rather than a general expectation that an atypical medicine will behave in one predictable way.

Names within a generation can conceal important differences. Some agents bind broadly across several receptor systems, while others have more selective profiles or partial agonist activity. Their metabolism and active metabolites also differ. An accurate comparison therefore specifies the medicine and formulation, using the generation as background information rather than the main explanation of expected benefits or tolerability.

Further reading: DailyMed prescribing information (1); DailyMed prescribing information (2).

Examples include risperidone, olanzapine, quetiapine, clozapine and lurasidone.
