It is used for several indications depending on the formulation and country. Its receptor pattern differs from that of conventional D2 blockers, including in its tendency to produce less prolactin elevation.

A partial agonist activates a receptor less strongly than a full agonist under comparable conditions. Several antipsychotics use partial agonism at dopamine D2 receptors alongside other actions. This distinguishes them from conventional D2 antagonists, but does not make them free of adverse effects or automatically preferable for every person with psychosis.

Possible adverse effects include akathisia, sleep disturbance, nausea and other effects. Weight change can still occur. Product information also warns about compulsive behaviours such as gambling or excessive shopping, which should be discussed promptly with the prescriber if they develop. The medicine has an active metabolite and is affected by drug interactions. Its activating reputation does not mean everyone feels more energetic or avoids sedation. Benefits and harms require individual review, particularly after starting treatment or changing exposure through another medicine.

New urges or changes in behaviour should be discussed explicitly because a person may not connect them with medication. Product warnings about impulse-control problems are separate from ordinary concerns about psychotic symptoms. Recognising the timing and impact can support review. The presence of such a warning does not mean everyone develops the effect, but it makes open questioning clinically useful.

Further reading: DailyMed prescribing information (1); Cariprazine prescribing information; DailyMed prescribing information (3).

Aripiprazole is an antipsychotic with partial agonist activity at D2 and 5-HT1A receptors and antagonist activity at 5-HT2A receptors.
