Cariprazine has particularly high affinity for D3 receptors and long-lasting active metabolites. Brexpiprazole also affects serotonin and adrenergic targets. These differences mean the medicines should not be treated as interchangeable versions of aripiprazole.

A partial agonist activates a receptor less strongly than a full agonist under comparable conditions. Several antipsychotics use partial agonism at dopamine D2 receptors alongside other actions. This distinguishes them from conventional D2 antagonists, but does not make them free of adverse effects or automatically preferable for every person with psychosis.

Adverse-effect patterns include varying risks of akathisia, weight change and sedation. Cariprazine’s metabolites can make changes unfold over an extended period, so immediate impressions may not represent the eventual effect of a dose change. Some clinical evidence examines negative symptoms with cariprazine, but that does not establish a universal benefit or prove that D3 action alone explains outcomes. Approved indications vary by jurisdiction. Comparing options requires the exact drug, treatment goal, previous response and physical health profile rather than the partial-agonist label alone.

Long-lasting metabolites can delay both the full effect of a change and recovery from an unwanted effect. This is particularly relevant to cariprazine. An early review may therefore need follow-up rather than a rapid series of adjustments. The appropriate pace depends on symptoms and tolerability, with the prescriber considering persistence as well as the nominal daily dose.

Further reading: DailyMed prescribing information (1); Cariprazine prescribing information; DailyMed prescribing information (3).

Brexpiprazole and cariprazine are D2 partial agonists with different additional pharmacological properties.
