Exact definitions differ, and adequate means more than having received a prescription. The medicine must have been used at an appropriate exposure and for sufficient time, with evidence that treatment was actually delivered.

Treatment resistance describes persistent clinically significant symptoms despite appropriately delivered treatment. In schizophrenia, the term usually concerns inadequate response to adequate antipsychotic trials, but definitions require attention to exposure, duration and outcomes. It should not be applied simply because recovery is incomplete or because one medicine was stopped early due to adverse effects.

The main symptom domain and degree of impairment should be specified. Persistent primary negative symptoms are not identical to persistent psychosis, and the evidence for further medication differs. Assessment also considers whether adverse effects prevented an adequate trial. The term describes a clinical pattern rather than personal stubbornness or unwillingness to recover. It is useful when it leads to a structured review and appropriate options, not when it becomes a label that prematurely ends efforts to understand a person’s remaining difficulties.

Resistance is defined relative to a treatment and an outcome. Persistent hallucinations despite adequate antipsychotic trials are different from ongoing cognitive difficulty after psychosis improves. Naming the target symptom prevents broad conclusions that every part of the illness is untreatable. It also helps connect the next clinical decision with the evidence relevant to that particular remaining difficulty.

Further reading: Open-access research; Research record 3046553.

Treatment-resistant schizophrenia generally involves significant symptoms that persist after at least two adequate antipsychotic trials.
