Possibilities include inconsistent access, missed doses, interactions, rapid metabolism, ongoing substance effects or a diagnosis that needs revision. A medicine may also have been abandoned because its adverse effects outweighed an emerging benefit.

Treatment resistance describes persistent clinically significant symptoms despite appropriately delivered treatment. In schizophrenia, the term usually concerns inadequate response to adequate antipsychotic trials, but definitions require attention to exposure, duration and outcomes. It should not be applied simply because recovery is incomplete or because one medicine was stopped early due to adverse effects.

The purpose of assessment is to clarify these factors without blame. Asking about practical barriers and subjective effects is often more useful than assuming non-adherence. Some people need a different formulation or better support with prescriptions; others need a different clinical explanation. Continued symptoms can also reflect depression, trauma-related difficulties or physical illness. Identifying an alternative contributor does not make distress less real. It creates an opportunity to adjust care before exposing someone to additional medication burdens or concluding that all standard options have genuinely failed.

The word pseudo-resistance is sometimes used for apparent non-response caused by inadequate exposure or another remediable factor. It should be used carefully because the person’s symptoms remain real. Identifying the contributor is an opportunity to improve care, not a reason for blame. The resulting plan should address the barrier directly and then reassess the relevant outcomes over time.

Further reading: Open-access research; Research record 3046553.

Apparent resistance occurs when symptoms seem unresponsive but another explanation limits the treatment trial.
