Clinicians consider other neurological and physical causes as well as the medicine’s benefits. Structured observations can help distinguish slowing and rigidity from reduced motivation, depression or sedation.

Parkinsonism describes a pattern that can include slowed movement, rigidity and tremor. Antipsychotics can produce drug-induced parkinsonism through dopamine receptor effects, but other causes must also be considered. The syndrome is not identical to Parkinson’s disease, and its appearance requires assessment rather than automatic attribution to psychiatric symptoms or ordinary ageing.

Management may involve a supervised dose reduction, switching or other selected treatment. The choice depends on the psychiatric situation and physical risks. Medicines sometimes used for movement symptoms can themselves affect memory, constipation and other functions, so adding them is not automatically the best solution. Follow-up determines whether symptoms improve and whether psychosis remains adequately controlled. The person should receive an explanation of likely causes and what changes to report. Practical support can reduce immediate difficulties while preserving autonomy and avoiding unnecessary loss of participation in daily activities.

The review should separate movement-related slowing from sedation and reduced motivation because the management implications differ. It can also examine medicines prescribed for other conditions. A plan is more useful when it specifies what change is expected and when to reassess. Practical support can preserve participation during this process rather than requiring the person to wait without assistance for a diagnostic conclusion.

Further reading: Australian medicine information; DailyMed prescribing information.

Assessment of possible drug-induced parkinsonism includes a movement examination, treatment history and evaluation of functional impact.
