Blocking these receptors can produce sleepiness, which helps explain the sedating properties of several antipsychotics. The same broad receptor system is also relevant to appetite, although sedation and weight gain do not always occur together in the same degree.

Sedation is reduced alertness or increased sleepiness associated with a medicine or another cause. Several antipsychotics can produce it, partly through histamine receptor effects. Its intensity varies with the drug, exposure and individual factors. Sedation can sometimes feel helpful at night while becoming a substantial barrier to daytime activity and functioning.

A receptor mechanism does not fully predict the person’s experience. Exposure, other targets, sleep history and interacting substances matter. A drug with relatively little H1 affinity can still cause somnolence through other processes, and a person may adapt partly to sedation over time. Clinical review should ask about actual alertness and functioning rather than rely on a receptor chart. The mechanism explains a possible contribution while leaving room to investigate other causes of persistent tiredness or impaired concentration during treatment.

Central H1 blockade affects wakefulness, but a drug’s overall sedating effect also reflects other actions and its concentration over time. This is why laboratory affinity does not provide a precise timetable of sleepiness. Observing when alertness changes after treatment and across the day is more informative for practical decisions than assuming a receptor label predicts the entire experience.

Further reading: DailyMed prescribing information (1); DailyMed prescribing information (2).

Histamine contributes to wakefulness through H1 receptors in the brain.
