It can interfere with reading, conversation, study and work. Antipsychotic effects are one possible cause, but insufficient sleep, sleep apnoea, irregular schedules and other medicines can also contribute.

Sedation is reduced alertness or increased sleepiness associated with a medicine or another cause. Several antipsychotics can produce it, partly through histamine receptor effects. Its intensity varies with the drug, exposure and individual factors. Sedation can sometimes feel helpful at night while becoming a substantial barrier to daytime activity and functioning.

Assessment asks when sleepiness occurs, how much sleep the person gets and whether there are signs of a sleep disorder. A person may spend a long time in bed without obtaining restorative sleep. Observing only total sleep hours can therefore be misleading. Safety-sensitive activities need particular consideration. Review can identify whether treatment changes or investigation of another cause is appropriate. The person should not be labelled unmotivated solely because they struggle to remain alert. Understanding the pattern helps protect both functioning and the benefits of necessary psychiatric treatment.

Sleepiness can fluctuate with activity and environment. Someone may stay awake during a stimulating conversation but struggle during reading or travel. Asking about several situations provides a better picture than a clinic observation alone. Unintended dozing, especially during activities requiring attention, is important to discuss because it can indicate a substantial functional or safety problem needing assessment.

Further reading: DailyMed prescribing information (1); DailyMed prescribing information (2).

Daytime sleepiness is a tendency to become drowsy or fall asleep during waking hours.
