Two people hearing similar words can experience very different levels of fear. The relationship with the voice, including perceived obligation to obey it, can therefore be an important target.

Cognitive behavioural therapy for psychosis is a collaborative psychological approach to understanding experiences and reducing their distress or impact. It explores relationships between thoughts, feelings, bodily responses and actions. The aim is not to argue someone out of experiences or demand acceptance of a particular diagnosis before useful work can begin together.

Work may involve coping strategies, attention, emotional responses and testing predictions in carefully planned ways. The person chooses goals, which might include better concentration, less distress or more freedom to act independently. Treatment does not require pretending the voice is unimportant or denying how real it feels. Safety needs are assessed when commands involve harmful actions or the person feels unable to resist. Therapy can complement medication and practical support, with outcomes judged by lived impact as well as whether voices are still present.

Voice-related goals can be behavioural and concrete. A person may want to concentrate for a short task, leave home or feel less obliged to respond. These outcomes can change even if voices remain audible. Recording them gives a clearer picture of benefit than frequency alone and keeps treatment connected to the activities and independence the person values most.

Further reading: NICE clinical guideline; National Institute of Mental Health; Research record 26360083.

Therapy for distressing voices may examine their content, triggers and the beliefs a person holds about their power or intentions.
