They may include grounding, structured activity, attention shifts, relaxation, problem-solving or seeking support. Therapy examines which strategies actually help and which provide brief relief while increasing longer-term difficulties, such as excessive checking or complete avoidance.

Cognitive behavioural therapy for psychosis is a collaborative psychological approach to understanding experiences and reducing their distress or impact. It explores relationships between thoughts, feelings, bodily responses and actions. The aim is not to argue someone out of experiences or demand acceptance of a particular diagnosis before useful work can begin together.

Strategies should fit the person’s preferences and circumstances. A method that helps one voice-hearer may be distracting or distressing for another. Testing one manageable change at a time can make effects clearer. Cognitive difficulties may require written reminders or shorter tasks. The goal is a broader range of choices rather than one prescribed response to every symptom. Coping work also recognises practical barriers, including unsafe housing or lack of support, which cannot be solved solely by changing thoughts or practising a technique.

Coping can have short- and long-term effects. Avoiding a place may reduce immediate fear while limiting valued activities and preventing new information. Another strategy may improve concentration without changing symptoms. Reviewing both time scales helps identify what is useful. The person’s preferences guide the balance, and genuine danger or practical barriers should be addressed rather than reframed automatically as avoidance.

Further reading: NICE clinical guideline; National Institute of Mental Health; Research record 26360083.

Coping strategies are actions used to manage experiences and their consequences.
