In family work, it can address appointments, household responsibilities, money or conflict about support. The approach works best when the problem is specific enough to act on.

Family interventions support people with psychosis and those close to them through education, communication and practical problem-solving. Family can include relatives, partners or other chosen supporters. Involvement should respect the person’s preferences, privacy and relationships rather than assume that every family arrangement is supportive or that relatives caused the illness.

Participants consider each person’s needs and choose manageable steps rather than seek a perfect solution immediately. Cognitive difficulties, symptoms and practical resources influence what is realistic. The person receiving care should retain a meaningful role in decisions. Review asks what helped and what needs adjustment without treating an unsuccessful plan as personal failure. Some problems require professional, financial or safeguarding assistance beyond family discussion. Recognising those limits prevents relatives from being expected to solve every difficulty and helps connect the family with appropriate external support.

A problem-solving review can ask whether the agreed step was clear, achievable and resourced. If it did not work, the explanation may lie in the plan rather than motivation. This approach supports learning instead of criticism. It also allows a family to identify when a difficulty requires outside services, professional advice or changes in resources beyond what household cooperation can accomplish.

Further reading: NICE clinical guideline; National Institute of Mental Health.

Structured problem-solving breaks a difficulty into a clear issue, possible options, an agreed action and a review of the outcome.
