Light, activity and social routines help align sleep with the day. Some people with schizophrenia experience delayed or irregular timing, making conventional work or appointment schedules difficult.

Sleep supports attention, emotional regulation and physical health. People with schizophrenia can experience insomnia, irregular sleep timing or excessive sleepiness, with several possible causes. Sleep disturbance is not simply an inevitable feature of the diagnosis, and a long period in bed does not necessarily mean that sleep is restorative or appropriately timed.

Circadian disturbance differs from simply not wanting to get up. Assessment examines when sleep naturally occurs, consistency across days and exposure to relevant cues. Medication, unemployment and limited daytime activity can influence the pattern. Treatment may involve structured routines and other clinician-guided approaches suited to the specific problem. Timing interventions are not identical for every sleep difficulty. Understanding the rhythm helps avoid advice that increases time in bed without improving alignment. The outcome should include whether the person can participate in the activities and relationships that matter to them.

The sleep–wake schedule reflects both an internal rhythm and opportunities imposed by the environment. A person may sleep well at a delayed time yet struggle with an early appointment. Identifying this pattern matters because the intervention differs from treatment for repeated awakenings or breathing-related disruption. Timing, regularity and daytime functioning should be considered together when evaluating progress.

Further reading: National Institute of Mental Health; Research record 26363701.

Circadian rhythms are approximately daily patterns in physiology and behaviour, coordinated by internal timing systems and environmental cues.
