Helpful plans are tailored to the person’s actual difficulty and circumstances. A stable waking time, manageable daytime activity and attention to caffeine or other substances may be relevant, but generic advice alone may not resolve a persistent disorder.

Sleep supports attention, emotional regulation and physical health. People with schizophrenia can experience insomnia, irregular sleep timing or excessive sleepiness, with several possible causes. Sleep disturbance is not simply an inevitable feature of the diagnosis, and a long period in bed does not necessarily mean that sleep is restorative or appropriately timed.

Assessment identifies whether the problem is insomnia, circadian timing, excessive sleepiness or another condition. Psychological sleep treatment, medical investigation or medication review may then be appropriate. Rigid routines can be difficult during acute illness, so goals should be achievable and reviewed. The person should understand why a change is suggested and what outcome it is intended to improve. Treatment success includes sleep quality and daytime functioning, not simply longer sleep. Persistent or worsening problems deserve reassessment rather than repeated advice to try harder at the same routine.

Consistency is often easier when the plan fits existing responsibilities and resources. Noise, housing, shift work and caring duties can limit what is achievable. Addressing these conditions may be necessary before behavioural advice can help. Sleep treatment should therefore connect biological timing, habits and environment while evaluating outcomes that matter, including alertness, comfort and participation during the waking day.

Further reading: National Institute of Mental Health; Research record 26363701.

Sleep routines provide consistent cues around waking, activity and winding down.
