CBD has different pharmacology and does not produce the same typical intoxicating effects. Products vary widely in their content, and labels or informal descriptions may not provide reliable information.

Cannabis contains compounds with different effects, particularly THC and CBD. Its relationship with psychosis depends on factors such as exposure, potency, individual susceptibility and the outcome studied. Cannabis is not the explanation for every psychotic illness, but it is a clinically important factor to assess respectfully and accurately rather than through blame or reassurance unsupported by evidence.

The distinction does not make CBD a proven replacement for antipsychotic treatment. Research preparations, doses and quality controls differ from many commercial products, and interactions or adverse effects remain possible. A product containing CBD may also contain THC. Assessment should identify what was actually used rather than assume that all cannabis products are equivalent. Understanding composition helps interpret experiences and risk, while recognising that an individual’s response also depends on susceptibility, other substances and the circumstances in which the product was taken.

Potency describes concentration, while actual exposure also depends on the amount used and delivery method. A product’s name does not reliably capture either. This matters when comparing experiences or research findings. Clear questions about the specific preparation and pattern of use are more informative than treating occasional low exposure and frequent high-THC use as identical categories.

Further reading: National Institute of Mental Health; World Health Organization; National Institute on Drug Abuse.

THC is the main intoxicating component of cannabis and can alter perception, memory and threat interpretation.
