It can coexist with schizophrenia and may be shaped by distress, social circumstances or established habits. A diagnosis does not describe the person’s character or motivation to improve.

Alcohol affects the brain, sleep, physical health and behaviour, and can interact with psychiatric treatment. Its relevance depends on the amount, pattern, timing and individual circumstances. A person with schizophrenia may use alcohol for many reasons, so assessment should be specific and non-judgmental rather than assume that any use has one explanation or effect.

Assessment identifies goals, dependence severity, physical health and withdrawal risk. Support may include psychological treatment, practical assistance and appropriate medical options. Safe withdrawal planning is distinct from maintaining longer-term change. Coordination between services can reduce conflicting advice and missed care. The person’s reasons for drinking should be understood so alternatives address actual needs. Progress may be gradual, and setbacks should prompt review rather than exclusion. Effective care recognises the interaction between alcohol, symptoms, medicines and everyday life while maintaining a respectful relationship and clear pathways to help.

Longer-term support can address triggers, routines, relationships and access to rewarding alternatives as well as physical dependence. Medical and psychological options have different roles, and the plan may change over time. Integration with psychiatric care reduces the risk of contradictory advice or exclusion from one service because another condition is present. Continued contact supports reassessment when circumstances or goals change.

Further reading: NICE clinical guideline; National Institute of Mental Health; NHS health information.

Alcohol dependence can involve impaired control, prioritisation of drinking and continued use despite harm, often with tolerance or withdrawal.
