Aerobic exercise, such as walking, cycling or swimming, can improve it when appropriately planned and repeated. Better fitness can make daily activities easier, although the response depends on health, intensity and consistency.

Exercise is planned physical activity intended to improve or maintain aspects of fitness or health. It can support wellbeing and physical health in people with schizophrenia, with benefits depending on the activity, consistency and individual circumstances. It complements clinical care rather than replacing necessary treatment or establishing a simple cure for psychosis or negative symptoms.

People with schizophrenia may face additional barriers through sedation, smoking, limited resources or reduced motivation. Assessment should consider these factors without assuming inability. A manageable starting point and an activity the person likes can support participation. Physical symptoms or relevant conditions may require medical advice before changing activity substantially. Progress can be measured through practical improvements, such as more comfortable walking, rather than comparison with athletic standards. Exercise planning is most useful when it fits ordinary life and preserves safety while building opportunities for sustained movement.

Fitness is specific to the activity and demands involved. A person may improve walking endurance without becoming comfortable with intense exercise, and that gain can still support daily life. Gradual progression and suitable recovery help make participation sustainable. The chosen measures should reflect meaningful changes, such as easier travel or less exertion during ordinary tasks, rather than comparison with athletic performance.

Further reading: NICE clinical guideline; Research record 25805886; Research record 36047035.

Cardiovascular fitness concerns the ability of the heart, lungs and circulation to support sustained activity.
