A person may understand the benefits and still struggle to participate. Treating inactivity as simple unwillingness overlooks both illness-related difficulties and practical constraints.

Exercise is planned physical activity intended to improve or maintain aspects of fitness or health. It can support wellbeing and physical health in people with schizophrenia, with benefits depending on the activity, consistency and individual circumstances. It complements clinical care rather than replacing necessary treatment or establishing a simple cure for psychosis or negative symptoms.

Assessment identifies which barriers can be changed. Options include shorter sessions, accessible locations, accompaniment, suitable equipment or a different activity. Medication effects and physical symptoms may need review. Support should offer choices rather than prescribe a demanding standard programme. Building a routine around something enjoyable can be more sustainable than relying on motivation alone. Progress may involve consistency at a modest level. Services also have a role in making opportunities welcoming and affordable, so responsibility is not placed entirely on the person to overcome every environmental obstacle independently.

A barrier-focused plan asks what makes the next session difficult and which change would help. Transport support, a quieter setting or a shorter activity may be more effective than general encouragement. Review should include whether the plan feels worthwhile to the person. Sustainable participation grows from feasible opportunities and appropriate support, with responsibility shared by accessible services and environments.

Further reading: NICE clinical guideline; Research record 25805886; Research record 36047035.

Barriers to exercise can include fatigue, sedation, movement symptoms, low motivation, anxiety, cost, transport and lack of safe spaces.
